Pay by Mail:

1. Complete the form online – the completed form will be emailed to you.
2. Print the form – mail it with a check payable to DAHPERD, ATTN: Membership and Conference Registration, PO Box 1639, Dover, DE 19903

Pay at the Conference:

  1. Complete the form online – the completed form will be emailed to you.
  2. Bring your completed form and a check payable to DAHPERD.

Just Want a Membership?

Complete the Membership only form (without attending the conference).

Already a Member?

Complete the Conference only registration form (for those who hold multiple year or retired membership). Valid membership required.

Notes:

♦ If you are expecting your district to pay for your membership, conference or both, YOU will need to pay upfront and DAHPERD will reimburse you upon receipt of payment from your district.
♦ Registration MUST be received PRIOR to 09/27/17 to be eligible for boxed lunch.
♦ Walk-Ins will be accepted on the day of the conference.
